The mission of Service Coordination of South Central Pennsylvania, Inc. is to assess the needs of individuals and recommend services which will enable individuals to live as independently and safely as possible in their own homes. SCSCPA will help to coordinate the needs of individuals with all available community resources. Through continual evaluations we will maximize and manage services to meet participants current and future needs.
Through Service Coordination, our professional staff will work with our participants, adults with physical disabilities, to identify and coordinate state funded in-home care services. We coordinate services through the Home and Community Based Services. Our dedicated Service Coordinators will make every effort needed to obtain, from local resources, those services necessary to achieve maximum independence for our participants in and out of the home.
Service Coordinators will help participants in setting up a plan of care. They will annually evaluate that plan and adjust it accordingly to meet their needs and goals. They will also contact participants' agencies of choice to provide their in-home care services. In-home care services support participants who need help to perform daily living tasks such as preparing meals, bathing/dressing needs, and/or transfers in and out of bed.
